Completing your patient registration forms before your appointment helps us prepare for your visit and keeps your time in the office focused on care rather than paperwork.
Please download, print, and complete the appropriate forms below. Bring the completed forms with you to your first appointment, along with any relevant medical or dental records. This will help reduce waiting time and allow your visit to begin promptly. If you are unable to print the forms in advance, our team will be happy to provide them when you arrive.
Registration Packets
Select the packet that matches your age group. Each packet includes registration, medical history, and consent forms needed for your first visit.
Adult Registration Packet
For patients 17 years of age and older.
Youth Registration Packet
For patients under the age of 17. A parent or legal guardian must complete and sign these forms.
No Printer? No Problem. If you cannot print these forms at home, simply arrive 15 minutes before your scheduled appointment time. Let our reception team know, and we will provide a physical packet for you to complete in our waiting area.
What to Expect at Your First Visit
Your first visit includes time for records, a comprehensive exam, and the type of cleaning that best fits your needs. Appointments are unhurried and structured to give you space to ask questions, understand your oral health, and discuss next steps with your doctor.
